Your Initial Consultation

Lasting as long as needed, this meeting provides the direction for your future care at VCRM. You will begin by meeting with your physician who will review your medical history, discuss your options and may even perform an ultrasound and or physical exam. Our financial coordinators will review your insurance benefits and financial options, and also can provide you with information about the cost of treatment.

To make this visit as effective and successful as possible, we ask that you submit the following materials at your first visit, so please plan to arrive at least 15 minutes before your initial appointment:

• Completed Demographic Form
• Copy of Your Insurance Card and Drivers License
• Completed Patient Intake Forms
• All Previous Medical Records
• HIPAA Forms
