Your Initial Consultation

Lasting as long as needed, your initial consultation provides the direction for your future care at VCRM. You will begin by meeting with Dr Sharara who will review your medical history and prior medical records, discuss your options, and may even perform an ultrasound and or physical exam. Our financial coordinators are available to review your insurance benefits and financial options (we will check your insurance benefits before you initial appointment), and also can provide you with information about the cost of treatment, should that be required. We strongly encourage you to bring a copy of your medical records along.

Please be advised that if you schedule an appointment and need to cancel or reschedule, we require at least a 24 hour notice . Failure to do so will force us to bill you for a full office visit. Missed appointments affect everyone. We have a waiting list for new patients, and we can easily use that time slot to accomodate another couple. Please be considerate and call the office to cancel or change your appointment. We work very hard to stay on schedule because we value your time and ours. Thank you for your understanding.

Please note that because of patients’ sensitivity and the swine flu epidemic, children are categorically NOT allowed in the office . Please make sure not to bring your children to the office.

To make this visit as effective and successful as possible, we ask that you submit the following materials at your first visit, so please plan to arrive at least 15 minutes before your initial appointment:

  • Completed Demographic Form
  • Copy of Your Insurance Card and Drivers License
  • Completed Patient Intake Forms
  • All Previous Medical Records
  • HIPAA Forms
